Are you currently taking natural desiccated thyroid? If yes, what dose did you start on, and what dose are you taking now?

Adrenal Support

Are you currently taking adrenal cortex extract? If yes, what dose are you taking, and what time of day do you take it?

Additional Supplements & Medication

Have you made any changes to your supplements or medications since your last submission? Please include anything you started, stopped, increased, or decreased.*

Basal Temperature

Have you completed one month of basal body temperature tracking? If yes, how has your temperature changed? Has it reached the second line, and has the variability changed?

If one test has multiple pages, combine only those pages into one PDF.

Use this format:

          FirstName-LastName-TestType-YYYY-MM.pdf
        

Example: Jane-Smith-Thyroid-2026-04.pdf

Basal temperature example: Jane-Smith-Basal-Temperature-2026-04.pdf
